1. Our Duty to Safeguard Your Protected Health Information (PHI) Yourframework.Health is required by the Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy of your PHI and to provide you with this Notice of our legal duties and privacy practices.


2. How We May Use and Disclose Your PHI

  • Treatment: We may use your PHI to provide medical care. For example, your provider may share your records with a pharmacist to call in a prescription.

  • Payment: We may use your PHI to bill you or your insurance plan for services.

  • Healthcare Operations: We may use your PHI for quality improvement, training, and business management.

  • As Required by Law: We may disclose PHI to public health authorities, legal entities in response to a court order, or to report abuse/neglect.

  • Electronic Communication: By providing your mobile number, you acknowledge that we may communicate with you via SMS for appointment reminders. While we strive to protect these communications, standard SMS is not fully encrypted.


3. Your Rights Regarding Your PHI

  • Right to Inspect and Copy: You have the right to access your medical records.

  • Right to Amend: You may request corrections to your records if you believe they are inaccurate.

  • Right to an Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI.

  • Right to Request Restrictions: You may ask us to limit how we use your PHI, though we are not always required to agree.

  • Right to Confidential Communications: You may ask us to contact you via specific means (e.g., "Do not leave voicemails").


4. Complaints If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at privacy@yourframework.health or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

5. Changes to Terms We reserve the right to change the terms of this Notice. Revised Notices will be posted on our website.
